Health Services Coordinator

What It Takes
This is highly skilled management work in the coordination and monitoring of resident services and activities; to promote the achievement of maximum levels of mental and physical health of the residents.  Assists in coordinating the educational activities and objectives of the nursing personnel and inservice education.

What It Takes
Degree as a Registered Nurse and / or Licensed Practical Nursing.

Valid license as a Registered Nurse and or Licensed Practical Nursing by the State of Ohio.

Three years’ experience working with the elderly, preferably in a nursing home or assisted living.

MDS Coordinator RN

MDS Coordinator

The Elizabeth Scott Community, a 5 Star, family-owned healthcare facility in Maumee, searching for a full-time, dedicated nurse RN with experience in preparing MDS assessments and care plans, working with the Medicare PPS and OBRA systems. Experience with MDS 3.0, RAI user manual and RUG categories preferred. Attention to detail and knowledge of current state, federal, and CMS regulations is desired.

Qualified candidates should possess strong clinical skills and be a team player. This position is responsible for the following:

  • Conducts and coordinates the development and completion of the resident assessment (MDS, care plan/nursing assistant flow sheets) in accordance with current rules, regulations, and guidelines that govern the resident assessment.
  • Maintains and periodically updates written policies and procedures that govern the development, use, and implementation of the resident assessment (MDS) and care plan.
  • Performs administrative duties such as completing medical forms, reports, evaluations, studies, etc. as necessary.
  • Develops, implements, and maintains an ongoing quality assurance program for the resident assessment/care plans.
  • Assists the resident and social services in completing the care plan portion of the resident’s discharge plan.
  • Participates in facility surveys (inspections) made by authorized government agencies.
  • Serves on, participates in, and attends various committees of the facility as required, and as appointed by the Administrator.
  • Provides written and/or oral reports of the resident assessment/care plan functions as requested or directed by committees.
  • Attends, participates, develops, and/or assists with staff development and training programs.
  • Performs Charge Nurse duties when scheduling difficulties arise.
  • Performs Medicare Utilization Review.
  • Serves as “on-call” person on a rotating basis.
  • Performs nursing duties as needed.
  • Perform related duties as assigned.

What It Takes
Your goal is to assist residents by providing responsive, high-quality nursing care. You will help with hygiene, comfort and safety, mobility, lifting, skin care, activities, nutrition and hydration. You will also assist nurses with treatments, rehabilitative procedures and hospice care, as well as record patient data.

What It Takes
To be eligible for this position, you must either be state-tested or test-ready (completion of a Nurse Aide Training course). Nursing students may also qualify for a position based on transcript eligibility.
